A calm, precise reading of the tirzepatide weight-management record — what the SURMOUNT and SURPASS trials actually measured.

Tirzepatide Research: What the Main Studies Found

Food may stay in the stomach longer

Food may sit in the stomach longer. Hunger often eases. Blood sugar may fall too. Tirzepatide research started with cells and mice. Human studies came next. Five large final studies, called phase 3, covered diabetes. This is the last test stage before approval. Five more covered obesity. Others studied sleep apnea, heart failure, and fatty liver disease. The newest reports came in 2024–2025.

Tirzepatide was the first FDA-approved medicine to do both gut hormone jobs. GLP-1 appears after food and helps the brain feel full. The insulin-helping hormone prompts insulin to leave the pancreas. Together they affect insulin, stomach speed, and hunger. Studies found larger changes with Tirzepatide than with GLP-1 alone. A source number follows each finding.

The study list can’t forecast your result.

Tirzepatide copies two hormones released after food

The drug is built from small protein parts. It copies two gut hormones released after food. Under GLP-1, the stomach passes food onward more slowly. The brain also gets a fullness cue. The other hormone helps the pancreas release insulin. Both act when blood sugar rises. The GLP-1 job is to slow the stomach and help with fullness.

The insulin-helping hormone also affects how the body handles fat. GLP-1 also holds down a hormone that raises blood sugar. These are separate jobs. Tirzepatide copies both. Its GLP-1 action works beside the insulin-helping action.

Lab workers put cells in a dish and compared GLP-1 with the insulin-helping job. They found stronger action through the insulin-helping hormone. The test also found a different kind of action through GLP-1. Scientists think that may help explain the added insulin release [2]. A cell test can’t show how much that matters in daily care.

A fat piece helps Tirzepatide travel with albumin, a common blood protein. About five days pass before half leaves. This supports a weekly shot [1][7].

The first small human study included 142 healthy adults and adults with type 2 diabetes. It checked safety and timed how long Tirzepatide stayed in blood. The drug lasted through the week. Placebo brought smaller drops in fasting sugar and body weight [1].

Cells can’t tell how the medicine will feel to you.

The SURMOUNT studies measured weight and breathing

SURMOUNT-1 followed 2,539 adults for 72 weeks. Obesity was present. Diabetes was not. Doctors used body mass index, a score made from height and weight. A score of 30 or more counted. So did 27 with another weight-linked health problem. Average change was -15.0% at 5 mg, -19.5% at 10 mg, and -20.9% at 15 mg. Placebo brought -3.1%. Most complaints were nausea or bowel trouble. They were usually mild or moderate and came during dose increases [4].

SURMOUNT-2 tested 10 mg and 15 mg for 72 weeks. Each adult had obesity and type 2 diabetes. Those taking Tirzepatide lost more weight than those on placebo [9].

SURMOUNT-3 began after a 12-week food and exercise plan. Its 579 people entered after losing at least 5%. The study tracked them for 72 more weeks. Tirzepatide brought another -18.4% change. Placebo brought +2.5%. Another 5% loss was reached by 87.5% with Tirzepatide and 16.5% with placebo [10].

SURMOUNT-4 asked what happened after people stopped. Weight returned when one group changed to placebo. Continued Tirzepatide kept more pounds off [13].

SURMOUNT-5 followed 751 adults for 72 weeks. None had type 2 diabetes. Average change was -20.2% with Tirzepatide and -13.7% with semaglutide. Semaglutide copies GLP-1 alone. Study math pointed away from a fluke. More people reached 10%, 15%, 20%, and 25% loss [5].

SURMOUNT-OSA studied serious sleep apnea with obesity. People had fewer breathing stops each hour as weight fell [12].

No group average can name your change.

Tirzepatide beat semaglutide in two direct studies

Two studies compared Tirzepatide with semaglutide. Semaglutide copies GLP-1, a gut hormone tied to fullness. Tirzepatide adds an insulin-helping hormone to GLP-1.

SURPASS-2 followed 1,879 adults with type 2 diabetes for 40 weeks. Three Tirzepatide groups used 5, 10, and 15 mg. The semaglutide group used 1 mg. A blood test looked back at several months of sugar levels. Drops with Tirzepatide were 2.01, 2.24, and 2.30 percentage points. With semaglutide, the drop was 1.86 points. These aren’t points on a home meter. Tirzepatide gave at least as much help at each amount, then showed the larger drop. Weight loss was -1.9, -3.6, and -5.5 kg greater [3].

SURMOUNT-5 followed 751 adults with obesity for 72 weeks. None had type 2 diabetes. Doses rose only while side effects stayed bearable. Average change was -20.2% with Tirzepatide and -13.7% with semaglutide [5]. Study math pointed away from a fluke. This was the largest direct study of the two drugs.

One study favored Tirzepatide on sugar. The other favored it on weight. Side effects also led more people to stop it [33].

You can compare the findings, but a direct study can’t choose your drug.

The 2024–2025 studies looked beyond weight

In 2025, one long study followed adults with overweight or obesity. Their lost weight stayed off. Fewer developed type 2 diabetes [36].

In 2024, researchers studied stomach side effects. Weight fell even among people with little stomach trouble. Nausea alone didn’t explain the loss [37].

In 2024, another report timed Tirzepatide in the blood. The result supported one shot each week [7].

In 2025, SUMMIT enrolled people who had obesity and a stiff, failing heart [11]. Their hearts could squeeze but were too stiff to fill with enough blood. In 2024, SYNERGY-NASH studied a fatty liver disease that can scar the liver. Doctors checked tiny liver samples and found less disease [35]. Also in 2024, SURMOUNT-OSA found fewer breathing stops during sleep [12].

No study can say which finding will matter to you.
